Rewards Not Given After Reviving Instantly, All Items Called Excalibur

I was with a team in Zabala, Eris for 30min. I revived right at the Same Time the rest of the team stepped on Extraction and i got NO rewards from the run. The Mission rewards screen first showed my stats, somehow ALL of my weapons/gear/warframe are called "Excalibur", no rewards present. When i went to check on the Liset the Last Mission Rewards it shows i was alone, didnt do any damage or killed anything, got no XP, everything just shows up empty.

I was farming for Neurodes and i checked if just in case i had gotten them anyway, but i didnt

The mission ended at around 12pm PST

As soon as it gave me the option to Revive, i clicked it. So seconds

I died on my way to Extraction so i reved as soon as it let me
