Having the same problem, mostly on SP void cascade
apparently this has been a problem since 2019, and it was supposed to be fixed "soon" early 2023
Whatever happened, the problem is back, and its infuriating when you're dealing with lvl 9999 enemies because when the armor strip fails the first 2 times, you often die to some eximus that can't even see you
This is extra bad because this is a very timing sensitive game mode, and you dying ( or just failing to kill the enemy under x amout of time ) will put more pressure on your teammates an dying often means the end of the mission for everyone, not just you
anyway, i'll try to detail my experience with the bug as best as i can;
I would say it fails 80% of the time at melee range ( 3~5 meters away from enemy )
Only fails like 20~30% of the time if i'm at least 10 meters away
When it decides to fail, IT WILL FAIL no matter how much you spam that ability... until it doesn't, usually around 2~3 seconds later
i know the shrurikens DID hit the enemy, because of the slash procs, so it's not that they're aiming for different enemies
Problem is probably how the augment triggers, and not the armor stripping itself, since primers work perfectly no matter the host/ping/range from enemy etc
