I did a nightmare mission on deimos horend, while playing Yareli and after finishing the mission I did not get one of the rewards, despite opening the vault (did not pay enough attention to know if it was the vault reward or the nightmare that did not come through). Since then every mission I start is telling me I have unidentified items, restarting the game fixes it until I play Yareli again. They don't actually ever identify as anything in the results screen.
