Appreciate the image, however even using the image as a source for getting an idea how it works. Then how does that explain objects that are not in that 150 meter 4 dimensional plane. Having objects be marked 250 meters in a straight line?

It has been "Fixed" in a way. Know it will locate "Rare" items but sometimes it decides whether or not it will locate them. Sometimes the icon does not show up and the item is around 200 meters from the player. Other times only 1 item shows up and there are other within 200 meters of that 1 item that is displayed. Other times it is a guessing game on where the rare item is located, as the marker does not give a direct direction where to go. It will just mark the target but not help in guiding the player to the target.

When using Orokin EYE for the Sanctum Anatomica missions, it does not want to register the items that are 'RARE'. It will be 50/50 on if it will display the 'RARE' items. Along with the ranges be very strange. Some times 125-250 meters to 500 meters, other times 800 meters to 900 meters. Along with it pinging 1 item or up to 4 at a time.
