Expected result: Should have normally loaded in without this screen.
Observed result: Got stuck on it and i was forced to quit the game.
Reproduction rate: It happened two times in a row. Barely rejoined my friend to start another mission then after finishing it, it happen again.

Can confirm this, very easy to reproduce (at least the "I'm stuck and must Alt-F4" bit): just hold tab to open (and keep open) the progress/rewards screen while you're traveling back to the flotilla (navigation, pick flotilla, animation starts, press and hold tab).

Can confirm. Polarize on Dark Split-Sword won't let me pick Umbra Forma or Stance Forma, just goes straight for the plain Forma.

I've had this happen and it didn't feel like a lot of loot had dropped. I guess all the resources that drop add up though.
Perhaps the culling algorithm could try a bit harder to keep wreckage and avionics around?

I got it to show up for me by setting Options -> Display -> Geometry Detail to "High" (halfway down the list).

On PC today I've run into an issue with the slingshot a few times now (on RJs from different hosts) where I end up in Archwing mode instead (can even pew pew with my Cyngas), but stuck to the ship, unable to do anything other than wait to be killed so I can revive myself and reset things. At this point I'm simply avoiding the slingshot.
Details: I enter the slingshot from aboard the RJ, sometimes it's fine, sometimes it bugs out. When I enter the bugged out slingshot-archwing mode I can't launch myself, there are no yellow target highlights, and left-clicking will shoot my archgun instead (no damage being done though). I show up as invulnerable but can still take damage. I can also use my archwing abilities. Using the omni tries to take me back to the RJ, but it just leaves me stuck in the same bugged mode. The only way out (without abandoning the mission) is to die and revive.

You can use the game's screenshot facility (F6 by default?) to find out what resolution the game starts out at.
Mine's set to 1280x800 but starts at 1024x768 until I alt-tab out and back in.

My screen natively is 2560x1600, I've set the game to fullscreen 1280x800, but when I launch it (through Steam), it starts at 1024x768. After alt-tab out and alt-tab back in, the game's resolution corrects itself.

I had no idea that Outbreak status UI was even there until I read your comment. Hidden, undiscoverable UI for the win.

Yesterday's Prime Time stream had the 3-day credits booster for its stream drop, and they've said that the next 3 Prime Time streams will have as their drops the 500 ducats, the display, and the glyph, respectively, to make up for the website being unreachable.

Would of course need to be localized, but it'd be nice if one could find all the elemental mods (90%, 60%/60%) with one keyword.

Something tells me you didn't look at the picture posted at the top that started this thread. According to that, the code should've rewarded both a glyph and a display.

Same, got the 500 ducats, booster, and display, but not the glyph.

Completed all tasks for last week, do see 5 new weekly and 2 new elite weekly tasks, and 2 completed daily tasks, but no new daily task for today.

I've checked it against my own bookkeeping for clan affinity and it now matches up again, keeping in mind that the trophies no longer grant affinity on first build, which apparently they're compensating for by granting affinity for the Obstacle Course Architect room.
In other words: it's fixed now.

In another thread, people mentioned that after they logged out and back in the recovered task started counting stuff again, but anything they had done so far was ignored.
So basically it looks like after older tasks are recovered, you'll want to log out and back in before trying to do them.

I suspect the bug was introduced when they added the trophies for Operation: Hostile Mergers.

Sigh. I wonder how much standing I've missed out on from all the Plague Star bounties I've started from an outpost.

I ran into this as well, in a fashion.
Full squad, except after using 4+4 we got a bit distracted and failed the mission before the mixer finished. Restarted from an outpost, much rejoicing when it was still at 4+4 when phase 2 started, except at the end we just got 1000 standing despite having defeated 4 Hemocytes and countless infested. The standing thing for Plague Star bounties started from outposts is apparently a different bug, but yeah, if you fail the mixing process, you should simply lose the stuff you put in and start from scratch on the next run through.
Seems like for now it's safest to just return to Cetus between runs.

That would explain what I've been seeing. Sometimes Shattering Impact works, sometimes it doesn't. Thinking back, it coincides with when I'm host vs. when I'm client.
