Golden Maw Glitches

I honestly don’t know what category to put this in, so I guessed it should go in general. If I’m wrong, at least fix these glitches, because they are literally making the game unplayable.

In my first encounter with the Golden Maw in the War Within quest, the Golden Maw was miles behind me, so I started to make my way to the next safe point. Just as I approached it, however, the Golden Maw magically appeared in front of me and ate me.

Shortly afterwards, I was standing on the last rock before the exit, not on any bones, and the Maw ate me, while I was standing on a rock.
