Controller input causing internal item changes

So I play with a controller, it still uses some older custom controls from way back. 

Normally things worked fine with a mixture of KB+M and controller. Things like Gear wheel I just use Num 1-9. 

But, now in fortuna, when I hot swap to the new stun spear for fishing. the moment the game takes a controller input it says I have the lanzo spear and I can't fish while playing with a controller. 

By default with my controller I don't have a gear wheel button even set so dunno how to fix this without possibly messing up years of learned controller habit.
