Random out of bounds respawns

Something that's been happening recently is I'll be running a mission, and randomly the game will decide that I'm out of bounds and respawn me.  The areas this happens are very much in bounds, and often in rooms that lack out of bounds areas entirely.  It initially started with elevators, so I chalked it up to really strange lag that caused me to phase through the elevator and into the empty shaft below.  Recently though it's been happening more and more, in random corridors.  Usually it spawns me in a different room entirely, though once it spawned me in the same room, and I was able to trigger the effect repeatedly.
