Lost Forma in "Actions" Page

I was trying to polarize a mod slot in my Aklex Prime, and I bought 3 Formas through the "Actions" page since I didn't have any at the time. Once I confirmed the purchase, the page closed and went back to the Aklex Prime's upgrade/mod page. It was Unranked, no polarities had changed, and when I checked I only had 2 of the 3 Formas I bought.

 

I'm not submitting this as a support ticket because it's not a huge loss, and I'd rather just rank it up again than wait a couple days for someone to look into it, but I figured it might be a bug somewhere worth knowing about.
