Hydroid's Undertow falls through the floor

Hydroid's Undertow falls through the floor for several seconds then pops you back up out of undertow form on surface.

Has happened a few times in the past, however just tried a nightmare Survival on Pluto and it occurred almost every time I tried undertow.  Made my Hydroid Prime unplayable.

I recently had the same issue on Wahiba, Mars. I was in one of the rooms separating the long walkway with the windows and the V slope and about as soon as I used the ability and slowly crept towards the door frame towards the walkway, I immediately began falling through the floor and inevitably had to abort the mission.
