I like the surprise of things...if they appear on alert when I'm on, its cool. If they appear when I'm off, I don't really care, it'll still pop out again in the future.

...on the other hand, the Nekros helmet doesn't add any stats and it's quite literally a cosmetic item...:v

If DE isn't lying to us, then it is randomly generated, although has some tweaks like items not being on an alert having a higher chance
