Sands of Inaros - Third Vessel

After placing the third vessel in front of the door, it opens as it should. However, after taking the sacred vessel that arises from the ground, the quest waypoint doesnt update and the tomb guardian doesnt spawn and nothing further happens except the waypoint becoming a yellow circle on the minimap, filling out the room. Nothing spawns and i can run around the map or meditate or do whatever. The guardian doesnt spawn. I have restarted this mission multiple times and its a repeating bug for me.
