Automatic Failure Even Though I Have Enough Revives

For some reason when I play a misson and get downed instead of asking me if I want to continue or forfeit, the misson goes straight to failure. I was playing a tier 2 extermination misson in the void when this happened. I still had all four revives and didn't die in any previous missons. And I was playing alone on private if that helps to figure out the problem. I don't know if this is an isolated incident or if this is happening to other players too.
