Every single time when I am fighting with Lich and using requiem mods (if the first mod went successfully all the others (2nd and 3rd one) are not showing up on the screen (successfully or not - doesn't matter). The screen/UI bugs out and stays like that until the end/extraction. Even if Lich is dead etc. Spoiler Edit: Seems this is more UI related, so feel free to throw it into the right forum section.
