Liches get Excalibur helmets on shoulder wrongly

Some of my Liches have been getting standard Excalibur helmets on their shoulder instead of what the helmet should have been.

It's not based on the helmet, as I had one Lich with a Nova Flux helmet on their shoulder, but since then I've created two Liches with the Nova Flux Helmet equipped and they both have Excalibur helmets.

I wondered if it was something to do with my Sentinel killing the Larvling but it still happened when I unequipped my Sentinel's weapon.

I'm not sure if it's something to do with being a client when doing the mission with the Larvling, as at least a couple of the times it has happened I've been in a game with someone else hosting.

I tried making a Lich with the Nova Flux helmet equipped while solo and this time it successfully has the Nova Flux helmet on its shoulder.
