Codex entries not getting revealed when scanned

Since I have nearly everything done you could do I started to complete the codex. First I recognized that with the sentient boarding parties during Scalet Spear. The entire squad is scann able but won't show up in the codex. Same for the sentient ramsleds. Back then I didn't care about them not being in the codex, but right now that I went back to scan everything, I noticed the same with the specters on the junctions specters. The difference to the sentients is that there are spots for the specters already in the codex but those don't get uncovered when scanned. 

It's no big problem gameplay wise but a mild annoyance for completeionist like me.
