The most consistent way of proc'ing the bug is going into Aegis Storm and opening Balefire then turning off Aegis Storm then turning off Balefire. The result is not being able to equip any weapons and melee-ing will pull out the weapon but it won't attack or block. Balefire also won't open for a while. The solution is either dying or waiting for Balefire to be activateable again and going Aegis Storm>Balefire>Balefire>Aegis Storm. I don't know what conditions it needs to unlock Balefire again but I would spam switching weapons with the operative.
