Broken state when doing liches

I was doing liches using Mesa + Ignis Wraith (no secondary/melee). Every once a while I would get into broken state where I can't use any weapon or ability. At first I suspected that stabbing thralls while ending 4th ability animation causes this, but later I got the same issue at random point of playing. The only way to reset this is to die (jumping out of ma bounds doesn't work).

I have some video footage with that issue.

EDIT: happens with melee equipped too
