Isolation Vault Requiem Cipher Bug: Incorrect Images Shown

Encountered a really nasty bug on the third Isolation Vault on a public run.

All 4 reactive crystals showed the same image.

The vault still showed 4 unique images like normal.

The crystals still acted as their respective, unique elements despite showing the same image. (We almost guessed the code blindly).

Another member of the squad reported seeing the same bug.

I had something similar. I somehow failed the T1 vault opening, but i am certain i did input the right sequence. Then onwards the T2 and T3 vaults automatically failed, without human input. The pictures on the crystals were i believe misplaced, but not duplicated.
