Anyone else seen this happen? Player controlled Necramech instantly fails and locks Iso vault door?

I was just trying to do the isolation vaults.  Someone had opened the hidden portal and had the wrecked playable necramech.  And apparently just goofing around, they charged the door as Loid revealed the code.  It instantly failed all three attempts and locked the door.  Has anyone else seen this?  Because if that's repeatable and not just a random one-off "what the hey?" glitch, that could be a problem.

It is because the Mechs deal void damage, so when the mech charged the door it hit 3 of pillars in the wrong order and failed.
