Question about Isolation Vault tiers

I experienced this multiple times already, but i'm not sure whether this is a bug or not.

So when i fail on Tier 3's second phase (the toxicity one), i go to Mother and take the Isolation Vault bounty again, and then i can just skip through phase 1 and go to where i failed, and the enemy are equally strong. But when i reached the Vault, there's only 1 Necramech and it drops Orientation matrices, not Animus. Do the Isolation Vault tier reset when you fail? Or i'm experiencing a bug?

Looks like DE didn't expect people from failing the vault.  The session always seems bugged whenever it was failed.

Worm cycle probably changed during your run, or it's a bug.

Skipping phase 1 is definitely a bug.

I wasn't aware whether the Worm Cycle changed or no since i'm spending most time underground...also, skipping phase 1 was consistently happening everytime i fail at 2nd Phase. Basically i take the bounty, Mother and Loid/Otak does their usual talk, and then without having to collect Fass/Vome Residues and making the Bait, Loid/Otak will instantly say that i can get inside the Infested Tunnel.

Success or failure of your previous bounty doesn't matter with regards to your problem of skipping phase 1.  That happens when you run two bounties in a row that use the same vault cave.

To correctly do the 1-2-3 vault runs you should be a different cave each time.  Each stage has a specific cave and there is a specific mother node that will give you the correct bounty.  The first bounty has three dots on the bottom.  The second bounty four dots and the third bounty five dots.  I figured all this out the hard way by making the same mistake you are making in not picking the "right" bounty for 2 and 3.

A sure sign that you picked the wrong bounty is if the number of mechs in the vault is not correct.  If you only see one mech then you are in the level 1 bounty.

I'm already doing so. I already cleared some Tier 3 runs alone. My problem is that sometimes when i fail at the 3rd tier once, it only spawns 1 Necramech in the 3rd tier's vault when i retried and succeed. Maybe it's the Worm Cycle changing, or maybe if you fail on Tier 2 and up, you have to do from Tier 1 once again, i'm not sure. And that's what i wanted to know.

You have to reset the map.   If you rerun the same vaults in the same session for whatever reason they will be broken (bypass doors, empty vaults, incorrect mech #'s). 

If you fail any part of it you should head back to the Necralisk to reset the map...

Not sure if is the same issue, but ... when I did my first vault run (solo so I didn't upset anyone with my lack of knowledge), I started T1, killed the mech but didn't open the vault (forgot, lol). Went back to the surface and got a new bounty from the mother located at the Abscess, it skipped the first collecting and also the mixing phase. Sent me back to the same cave and I simply went straight to the toxic 3 minute phase. I killed the mech (vault only had 1 mech) and remembered to open the vault this time.
