Tyl Regor's axe is black & untextured and his fists go missing during the boss fight

The axe is black and untextured both in normal gameplay as well as in the Codex and even on his Noggle

Also, after he fires his fists at the ceiling for the first time during the boss fight, they will not show up back on his arms again

This bug is still in place as of update 29.5.2.
