I am not sure the conditions to produce this, but it appears every so often I am missing the Rot C reward. I have been hacking all three vaults undetected each time. Same weapons, frame, companion, etc. The only variable factors would be how many enemies killed, how many abilities used, and how many times the general alarm was activated.
