Forma caused crash, wiped my config

I used a forma on my Gara Prime to polarize her exilus slot (to Zenurik, if it matters). As soon as the forma was applied, my game froze. The screen would move or tilt with my mouse movements but I could not click on anything or use any keyboard input. I X'd out of the game (since I couldn't hit escape). When I relogged, my entire Gara Prime config was wiped.
