Missing Axi relic reward in Mot Steel Path

I did a 60 minute Mot survival on Steel Path.  This mission rewards an Axi relic 100% of the time at C reward (20, 40, 60 min).  I should have gotten 3 Axi relics but instead I only got 2 (see screenshot).  Obviously missing 1 Axi relic is no big deal for me, but it's clear that something went wrong and I wanted to let you know.

I think the problem happened at the 20 minute mark and I was using Octavia Prime on solo mode.
