Disappearing Mods?

Im not sure how it happens, but suspiciously my vigor mod is missing and I had some mods disappear in the past. Does anyone know what causes this?

 

One thing I can think of is region switching. I switch regions a lot, and my theory which shouldn't even be taken with a grain of salt as I know nothing of networking or how the games network is structured. But I assume i upload my stats and mods to the server region and when I switch regions, the other region hasn't been updated with my mods and stats and then uses an older profile of my stats and mods and then later updates all the regions with my "outdated profile". Thus losing my true recently earned or traded mods and stats.

 

Any thoughts?

Same here.

Lost 2-3 Speed Holsters, Sniper Rifle Ammo Mutation (from event, not farmed; it's gone. I didn't use it or fuse/trade, but i couldn't find it) and something else, uh, can't remember. Don't know about region switching, but they're just disappeared from mods (and it's not a bug in Codex). Tickets to TS are useless.
