Orokin Vault Missing?

A friend and I were running an orokin derelict extermination mission hoping to find a vault(We both had different keys equipped). We searched around "slowly" and "cautiously" the entire map to find it but we had no luck. Is this a bug or is there something I am unaware about?

The Vault is not guaranteed to spawn.

Actually it is, there was a glitch that caused it but it was hotfixed way back in U10.

I forgot to add that just take your time with these vaults. They may not appear when you want them to. And I advise you to play with 4 players that all have different keys just so if there IS a vault, you have the correct key.
