Earlier I was playing on Kappa and I used Hydroid's Undertow (puddle). When the ability ended, I fell to the ground as if I was downed, but I wasn't. I still had health and I could use my abilities (which was funny, because I would stand up, use the ability, then fall over again). His tidal wave ability came in useful when actually moving about. When I'd had enough of this, I decided to wait until I was killed, then revived, and I could stand again.

 

This is what it looked like:

Later on, at extraction, I used Undertow again (as you might), then I noticed something extremely peculiar - in the centre of the puddle, there was a tiny Hydroid with no head!

 

Here you can see him (he's underneath Carrier):

I have no doubt the first bug has occurred before, but I assume the second bug is exclusively Hydroid's.

The first bug has happened to me on Volt a few times. I ressurect my friends while I am crawling on the ground.

It's a bit funny but it gets annoying fast.

 

The second happens to me all the time, I spend like 40% of my puddleform being a tiny pirate baby.

I wish puddle form was a whirlpool instead.

I've had the 2nd bug before, and to be honest, I hope they make it a feature not a bug, because the mini Hydroid is awesome (you can even walk around a little bit) :D

I would love for it to be a whirlpool.  I'd also like it if you could zoom out a bit while in puddle form; trying to keep an eye on things from ground level is teh suq.
