Few Random Things? Missing Rewards And Energy Drain

I had a few questions about odd things if they're known issues, or maybe I just don't understand something.

 

I was doing some normal run of the mill mission and I happened to glance down to notice the last of my energy draining away.

I had energy siphon aura, so when it hit 0 it just recharged back up.

there were no disruptions of any kind -- mob or door, and this was not a nightmare mission.

 

me and a buddy did a nightmare mission where we each died during the course of it -- he ended up getting a nightmare mod as a reward at the end, while I just got the mats but no mod --- does it give you a mod every time?

 

on another nightmare I killed an ice leader guy via melee and my buddy noticed a purple ball had dropped.

when he grabbed it he said it was an oberon hat bp, but neither of us got it in the rewards list at the end of the mission.

 

do any of these sound like known bugs?

the mods should show up towards the top but, oberon helms aren't high priority so if the rewards thing is full it may not show up
