Kubrow Not Spawning, Losing Loyalty

I've encountered a strange bug today that is highly frustrating.

 

I began the day with my Kubrow sitting comfortably at 80% Loyalty. During a Corpus mission I spawned in a small closet within the ventilation system, however my Kubrow was conspicuously missing. At the time I thought nothing of it as my HUD indicated that he was well. I simply assumed he was stuck outside the map and proceeded as normal. After the mission I proceeded to upgrade my equipment as usual, but when I double-checked on the status of my pet his loyalty had been reduced as if he had been killed.

 

This has happened multiple times today, and now my beloved Kubrow is at a mere 20% Loyalty after using all of my daily interactions.

 

Has anyone else had this problem?
