Boar Prime Build?

Right now i have a x3 Forma Boar P.

 

1V and 2 --.

 

What mods should i use and does it need another Forma?

try to get your hands on some event mods, none use shotguns these days so they're real cheap, all the dmg, multishot, mods of course too

I found Corrosive + Fire (w Ember) or Viral + Radiation are pretty good.  I don't think Fire Rate are needed.

 

Edit:  And Shotgun mutation is recommended.

The only Event mod i don't have is Shell Shock, i tryed a x4 Forma build on Warframe Builder, 12k DPS, is that good enough?

it depends on the faction you're facing, and a screenshot to you're build will be helpful...I'm not expert in DPS (I don't even know what it means XD) but it's seams good 

edit: I just made a build for it and got 18k+ burst dps
