Having Trouble With Voice Chat (Receiving)

I don't know when it started, but apparently I'm having trouble with receiving voice chat. The person I'm trying to hear isn't mute, my options have voice chat enabled, and voice receive volume set to max. It's also not just between me and this person I want to hear, but for others, as there has been conversations I've been missing out on since I can't hear anybody over voice chat.

AFAIK, it's a similar issue that text chat is having.  Sometimes you just lose connection, for no apparent reason.

It's not that I lose connection, it's that I can't hear anyone over voice chat. It's not random, it's every time.
