Successful Mission - Mission Failed

Have had this several times over the last few days. Would complete a mission, get the reward, but upon returning to the planetary map it pops up "Mission Failed" with no stats available, group disbanded, and mission counting against 'fails' on the stats. All rewards are still counted, but it's also counting as a Mission Fail as well. Most annoying on alerts, along with most common for me.

i have encountered this. i was leveling up some weapons for mastery at the time and this showed up saw they hit lv 30 was about to sell when i noticed it didnt save the xp progress on the weapons.
