Cephalon Fragment scans lost on death

I've had this happen a few times.  Running a mission (any type, solo) and I scan a Fragment.  I get the animation where you see what Codex entry it will unlock and how many fragments I have left to scan for that entry.  So all is good.  Then I die (last night playing as Limbo on Neptune and got swarmed by three Corpus Techs - but that's not really relevant to this bug report).  Since I've had this happen before I tagged the Fragment so I could find my way back to it.  I went back to the Fragment and tried to re-scan it, but was told that my target had already been scanned.  And yet when the mission ended I din't get the Codex entry.

So, I lost the Fragment scan upon death and couldn't re-scan it.

Seems wrong to me.  I'd prefer not to lose the scan on a death, but if I did at least let me go back and re-scan it.

If it matters, I'm using the Simaris scanner with all three widgets equipped.  This has also happened with fewer widgets equipped and (I think) even with the normal Codex scanner.

sounds like a glitch - just rerun the same mission and don't get killed......
