Survival fissure refined relic rewards

While doing a T1 fissure on V Prime (Venus) i found out that i was not receiving the appropriate refined relic rewards.Instead of the relic reward,the game showed a white box that said "Item Name" under it.In 140 minutes i only received 3 exceptional relics,not in the sequence they are supposed to drop,no flawless or radiant relics at all.Also there were no refined relics when i returned to the ship,so they just disappeared.On another occasion the game would reward the first exceptional relic after 6 relics have been opened instead of five.I havent played for a few days,maybe the relc rewards are now random?Any info will be appreciated.
