Riven will not open after mission

I have a veiled riven that states "Open 2 staches on your own." I have now completed the task four times and everytime I extract from the mission it resets. I'm not dying and my mission is set to invite only with just me in it.  I've tried no companion and that doesn't work either.

A couple of questions:

• Do you have the riven installed on a weapon that is equipped?
• Does the riven task pop up at the start of the mission (like the bonus affinity tasks that pop up)?
• Does it update when you find a cache?
• What does the exact text on the riven state?
• After you've found 2 caches, does it say the riven challenge is complete?
• If the above has happened, have you tried immediately aborting the mission once the popup tells you the riven challenge is complete?
