Valkyr glitches on survivals

Almost all the time when I run Survivals on Valkyr and now on Valk Prime, I get glitched out where I can run around, but can't use any abilities or weapons, rez people, or activate life support and all the loot my Carrier picks up just floats above my head. Looking at what I left the mission with, I didn't have a single mod, when I had 4+ floating above my head. Most recently it happened in the Forma BP mission on Rosalind, Uranus when, but it has happened many times previous and I only recall having the problem in Survivals. Also, dying helps for a short time before it happens again, and always seems to happen after Hysteria. Though even if I don't go into Hysteria again after the first time it glitched, it still will happen again.
