Mod Disappering

I had just finished running the Mars alert taking my time with my loki to farm the mission for my heat dagger. I got a few mods and started combining the hornet strike i got with my hornet strike i am leveling. I used just the one mod and hit yes and immediately clicked my rank 4 Heated Charge to combine it with the heated charge i got in the level and the Rank 4 mod disappeared. Im going re-log to see if it was just a visual mistake.  

edit: just re-loged and the heated charge that disappeared is back but its rank 3 now
