This usually depends on how good you were. In spy missions you get an extra exterminate if you get detected in all three vaults. Not sure if it's a condition or affects the probability. I haven't seen it in other missions so there might be other triggers like alarms or time taken.

That one is hardcoded; if you trip all three spy alarms, you are guaranteed to get a "exterminate all enemies" punishment

I suppose it is to maintain alertness on players, instead of thinking bringing in Ivara for spy mission 
with all unranked weapons to earn EXP, you are then assigned with exterminate which you didn't plan for 

and kind of let the slack of mind thinking it would be a breeze walk
when mission suddenly changed to something more challenging than your current inventory could handle. 

It also make sense if you trigger too many alarm in a spy mission that the enemy is likely to report the matter
and causes tighten in security measures, so exterminate sounds logical.

I understand why you would dislike it. I personally like it, it adds more interesting variance and unpredictability to the missions.

I wouldn't mind some variety in different missions, but forcing exterminate into a mission that is supposed to be quick just sucks and makes me want to abort the mission. I didn't come to that mission to waste time.

Also, no, it's not something that happens rarely, it actually happens often for me. I see no point in it staying.
