The Helios is also incapable of scanning allies and the Acolytes (could be that they are technically a Warframe and are seen as an ally?), and possibility hidden entries.
It doesn't help that the codex entry's name and the in mission name above the enemy can be different, such as with the Carrion charger eximus; which is named Sanguine Charger during Defection missions.
The Cascade bomb entry needs 3 scans to register fully, though you only have one chance to get it, unless you quit or fail the mission, as the quest is unrepeatable.
Then there is the separate Hyena pack entry, which according to some other threads can be achieved but has very specific perimeters for it to scan and can be sotrie only.
