I did yesterday the nightmare alert for Hammer Shot, but after completing it, the mod is nowhere to be found. I got the money reward but I can't say anything regarding the rest of the mods I picked up. Hammer Shot did show up in the after mission briefing where your rewards are shown.
Just a FYI that it is possible to run a mission and not get all the rewards. As far as I can tell, this is the first time that this has happened.

Yup, just got done with the Speed Holster alert. No mod :(.
