[Teleporting and falling through floors]

Currently in several missions i have had issues with falling through maps and Teleporting back to prior positions. This didnt bother me much till a subsequent worm hole was directly infront of the elevator during a nitian alert, preventing my team  mate and i from progressing, we tried everything we could think of.
