I'm currently downloading the newest update. If this issue is still present, I'm uninstalling the game until railjack hits. There are a dozen or so posts about this issue and nothing anywhere about a possible fix incoming. Seems DE doesn't really care when people miss out on items that can be otherwise bought with cash

I'm having this issue with mods. It seems to happen on just about anything: weapons, frames, pets, you name it. Sometimes it would revert to the mods that were on it before hand and other times it would just outright remove all of them.

Console has yet to receive the update that changes it from 5 to 3. It's going to come with the PoE rework update

I've had this since shortly before the update. It seems to only happen on endless missions like defense, except I still receive the reward from the last relic opened. Ive also had friends and alliance members with the same problem

Are you returning to fortuna after doing each fracture run? If you just abort to your ship it won't save how many you've done. You can confirm how many fractures you've completed by doing a fracture then returning to fortuna. The total number of fissures sealed will be listed on the left side where the mission time and credits received are
