Every now and then (roughly less than 10% of the time I've played) my icon on the minimap with rotate etc. normal but you won't see the icon progress through the room like the minimap should. This has happened mostly during the Mercury missions, though I haven't noticed if it has happened during any missions past that. Not sure if it was mentioned previously or not but figured I'd mention it as when it does happen you have no real way of finding the next objective and without assistance from the teammates you are lost on the map.
