Ground Slam / Falling Through Elevators

Just had a bug happen to a few times, using Rhino and used my jump-melee attack and fell through the floor. Fell until I fell out of the map and respawned outside the elevator.

 

My friends haven't reported this happening so I'm not sure if it's just rhino, just the furax (which I was using as a melee weapon), or if it's a glitch with the elevator.

I'm hoping that it's a glitch with the elevator. With my Ash, I was jumping around, slamming my dual ether, while the elevator was going up/down and suddenly I fell through and didn't actually respawn. I didn't receive any rewards and it was an alert ):

Someday ago I just jumped while into the elevator and I fell down too, and I don't remember if I respawned (I didn't, if I'm not wrong)

i was falling constantly in the elevator on Baal for like 2 minutes -_- map clipping error and connection issues were prolly the cause of similar  things.
