Dark Split-sword has multiple equipment entries

In the Profile screen, under Equipment and Melee (they still shows up under just equipment) the Dark Split-Sword shows three entries, all of which do not sync up to each other (rank wise).
There is "Dark Hybrid Sword - Dual", "Dark Hybrid Sword - Single", and "Dark Split-Sword"
For me, they show up as Max Rank, Rank 23, and Unranked (in that order)

(Do note that i utilized the heavy sword stances until around halfway through when i then switched to dual swords, which is most likely contributing to the rank becoming out of sync)

The codex, however, does not show the two extra entries and, only displays the "Dark Split-Sword".
Which shows up as Max Rank
