[Headless and missing attachments + closed doors]

I've seen this as well while playing with my Shade sentinel. When enemies come near my Warframe, Shade will make me invisible but when I turn visible again, my head is missing and it seems like Shade sentinel is floating lower than other sentinels. I've stopped playing with Shade thinking its a bug with that sentinel. I have not seen chopped off head since. I also update video drivers, just to be safe
