I guess you were too slow for your team and one of them used their operator whilst stood in the arena, possibly just for zenurik energy (or maybe on purpose). This makes it register 4 stood there and starts the fight. 

 

I believe dieing and reviving transports you into the arena. 

i tried respawning but it didn't help

I was doing a Sortie 3 Ambulas Boss fight and when i got to the boss arena (as the last player) my team had already entered the arena and i couldn't get in anymore,Video of the bug (only first few seconds necessary) because the door didn't open for any of us.
