Did you unequip it yourself or when you tried using it did it unequip it automatically? If the latter, I think it's normal.
I did it myself because if I didnt it flash red on the bar when I goto modify it and it did not unequip automaticly

I have no idea on where to put this but here goes nothing.
I recently traded a Gleaming Talon stance to someone while the mod was equipped on my potatoed Glave Prime at that point with my mod total at 63 out of 70. Today I equiped my glave prime and desided to do some modifications and it still had the mods on it. I did take a screenshot.

If you just want the condensed version:
To earn Tier 1 rewards, go 50% Grinner and 50% Corpus then do one more mission for the side you want the badge. (Not dependant on victor)
To earn Tier 2 rewards, get 25 missions done. (Item is determined by the victor of the event and you get a item slot with it)
To earn Tier 3 rewards, go 50% Grinner and 50% Corpus then do one more mission for the side you THINK will win and will determine if you get a supercharged weapon with item slot or just its blueprint.

My idea is add extractor deployer devices stop at a planet and deploy a extractor and move on to another planet
