Isolation Vault Progression Broken

I have completed numerous T1 Isolation Vaults every single time I go to the Mother hub next to the T2 vault I get the Message.: "You must complete Isolation Vault". I have done this solo in a group of 2, 3, and 4 with 100% repeatability. I have completed every quest and the only thing in the starchart that I haven't done is Empyrean. Every quest is complete and I can find no reason why this should happen. The only clue I have is that Mother says, "This is a difficult place to navigate with one eye open." when referring to the vaults and transference. This seems to imply that I haven't completed something in the story but I can't figure out what it is. Does anyone have any idea what's going on.
